There is a charge to use the medical questionnaire functionality, so please contact HART support in the first instance to get this activated for you.
Create the questions in HART
Log into HART and navigate to Configuration > Services > Spa > Questions.
Questions consist of 3 parts, Groups, Questions and Answers.
First, you create the question groups. In the below example, there are three groups, Conditions, Preferences and Covid-19.
To create each group, right-click in the Question Groups window and chose Add New Entry.
Once you have created your question groups, you can create the questions which go into each group. Select the group you are adding questions to from the top list, and then in the middle box add new entries for each of the questions.
There are 4 types of questions;
- Multiple Choice. Put the answer in the bottom part and the guest can select as many as are appropriate
- Single Choice. Put answers in the bottom part and the guest can only select one of these options. Ie Massage pressure.
- Text: Free type box to fill in. NB. If you chose this type you still need to put one answer at the bottom but can leave it blank
- Dropdown: The guest can select one answer from a drop-down. This is not so user-friendly for mobile so recommend using the multiple or single-choice options.
For each question, you then add in the bottom part the answers available.
Note: Each window is linked to the previous one. Ie, you need to ensure you have selected the correct group when adding your questions, and the correct question when adding your answers.
Now that you have completed this, you are ready to move onto building the online form.
Style and build the online questionnaire
Log into cloud reservations, and navigate to Facilities > Spa > Facilities. Select the spa facility you are creating the form for, and then select the Medical tab.
- Submission Text: The wording which will display when the guest completes the questionnaire
- Title Text: The header which will appear at the top of the form
- Submission Button Text: The text on the submit button
- Cancel Button: The text on the cancel form button
- Form Expiry: How many months the questionnaire will be valid for
- Return URL: Once the submission is complete, this is the URL the guest will be directed to
- Enable Signature: Turn on if you require a digital signature from the text
- Signature Text: If a signature is enabled, then this is the accompanying text
- Enable Opt-In: tick if you would like the marketing text to show on the MQ form.
Next, fill in the styling colours the form should take.
Using the medical questionnaire on a tablet
Log into https://cloud-reservations.net/SpaArrivals/Index/HOTELID (if you don't know your hotel ID, please contact us).
This will bring up all spa arrivals for the day (or selected date). You can review, or view the medical questionnaire for each guest by clicking on the Select button.
This could then be handed to a guest on a tablet to fill in when they arrive at the Spa.
Embedding the link to the questionnaire in a confirmation
When the guest confirmation is sent out for a treatment booking, you are able to embed the medical questionnaire as a link for them to fill in ahead of the say.
The link is as below, with your hotel ID inserted instead of the text.
